Can a Lone Star Tick Bite Cause More Than One Kind of Illness?

A lone star tick bite can lead to very different medical problems
Some involve infection, while others do not
The distinction can change what doctors look for

A lone star tick bite can be associated with more than one kind of illness. The important point is not simply that this tick is linked to several diseases. These conditions can develop through very different biological mechanisms.

For example, a lone star tick can transmit bacteria that cause ehrlichiosis. Its bite is also associated with Southern Tick-Associated Rash Illness (STARI), which can resemble early Lyme disease even though its cause remains uncertain. And a lone star tick bite can trigger alpha-gal syndrome, an allergic condition rather than an infection.

This means that developing symptoms after a lone star tick bite does not automatically point to one diagnosis—or even to one type of disease.

One tick species can be associated with very different illnesses

The lone star tick (Amblyomma americanum) is an aggressive human-biting tick found across a large portion of the United States. It is widely distributed in the Northeast, South, and Midwest and is associated with several bacterial and viral infections as well as conditions that are not established infectious diseases.

Three examples—ehrlichiosis, STARI, and alpha-gal syndrome—show why the distinction matters.

Ehrlichiosis is a bacterial infection

Lone star ticks can transmit Ehrlichia chaffeensis and Ehrlichia ewingii, bacteria that cause human ehrlichiosis.

Symptoms can include fever, headache, fatigue, muscle aches, nausea, vomiting, diarrhea, and sometimes a rash. Ehrlichiosis can become serious, particularly when treatment is delayed.

This is a conventional tick-borne infection: an infected tick transmits a bacterial pathogen to a person. Because ehrlichiosis is bacterial, antibiotics such as doxycycline are used for treatment.

That mechanism is very different from what happens with alpha-gal syndrome.

Alpha-gal syndrome is an allergy, not an infection

Alpha-gal syndrome (AGS) is an allergic condition associated with tick bites. In the United States, the lone star tick is the tick most often associated with AGS.

Alpha-gal is a sugar found in most mammals but not in humans. After certain tick bites, some people develop an immune response to alpha-gal. They may later experience allergic reactions after exposure to mammalian meat or other products containing alpha-gal.

Symptoms can include hives, swelling, abdominal pain, nausea, vomiting, diarrhea, difficulty breathing, dizziness, or anaphylaxis. One unusual feature is that reactions after eating mammalian meat can be delayed for several hours.

Not everyone bitten by a lone star tick develops alpha-gal syndrome.

Importantly, AGS is not a bacterial infection. Antibiotics therefore do not treat the underlying allergic condition.

STARI can produce a Lyme-like expanding rash

Southern Tick-Associated Rash Illness, or STARI, creates another diagnostic challenge.

Patients can develop a red, expanding skin lesion after a lone star tick bite. The rash can resemble the erythema migrans rash associated with Lyme disease. Patients may also report fatigue, fever, headache, and muscle or joint pain.

However, the cause of STARI remains unknown.

The Centers for Disease Control and Prevention states that studies have shown that STARI is not caused by Borrelia burgdorferi, the primary bacterium responsible for Lyme disease in the United States. Researchers have investigated Borrelia lonestari as a possible explanation, but its role has not been established.

A 2026 report in the CDC journal Emerging Infectious Diseases illustrates that uncertainty. Investigators described an 8-year-old child in New York who developed an erythema migrans-like skin lesion after a confirmed lone star tick bite. B. lonestari DNA was detected in the tick, but the organism remains a disputed cause of STARI.

The case is important because it demonstrates a practical problem clinicians and patients can encounter: an expanding rash can follow a lone star tick bite and resemble Lyme disease even when the cause of the illness remains uncertain.

The lone star tick is associated with other infections

Ehrlichiosis is not the only infectious disease associated with the lone star tick.

The CDC lists the lone star tick as transmitting Bourbon virus, Heartland virus, and the bacterium that causes tularemia, in addition to the bacteria responsible for ehrlichiosis.

That broader spectrum reinforces why the identity of the tick can sometimes help clinicians develop a differential diagnosis. A patient with fever and systemic symptoms after a lone star tick bite may require a different evaluation from a patient who develops an expanding rash or delayed allergic reactions after eating mammalian meat.

Does a lone star tick transmit Lyme disease?

Lone star ticks are not considered vectors for Borrelia burgdorferi, the primary bacterium that causes Lyme disease in the United States.

The confusion is understandable because STARI can produce an expanding lesion that resembles the erythema migrans rash seen in Lyme disease.

In regions where both blacklegged ticks and lone star ticks are present, identifying the tick can therefore provide useful information. However, patients do not always see or save the tick, and tick identification alone cannot determine whether a pathogen was transmitted or explain a patient’s symptoms.

This is different from a tick transmitting multiple infections

There is another important distinction. A single infected tick can sometimes transmit more than one pathogen. That is the problem of tick-borne coinfection.

The lone star tick illustrates a broader concept. The same species of tick is associated with conditions that fall into fundamentally different medical categories.

Ehrlichiosis is a bacterial infection. STARI is an illness associated with a lone star tick bite whose cause remains uncertain. Alpha-gal syndrome is an allergic immune response.

Those differences can affect diagnosis, testing, treatment, and what symptoms patients and clinicians should watch for.

Knowing which tick bit you can provide useful clues

Identifying a tick does not diagnose an illness, but it can change the list of conditions that deserve consideration.

After a lone star tick bite, fever, headache, muscle aches, gastrointestinal symptoms, or rash may raise concern for an infectious illness such as ehrlichiosis. An expanding Lyme-like lesion may raise the possibility of STARI or another cause of an erythema migrans-like rash. Delayed allergic symptoms after eating mammalian meat may suggest alpha-gal syndrome.

The patient’s geographic exposure, symptoms, timing, physical examination, and appropriate laboratory testing remain important. The tick species is one piece of that clinical picture.
